CHNA9 will operate in the 27 cities and towns of the North Central region of the state of Massachusetts (Ashburnham, Ashby, Ayer, Barre, Berlin, Bolton, Clinton, Fitchburg, Gardner, Groton, Hardwick, Harvard, Hubbardston, Lancaster, Leominster, Lunenburg, New Braintree, Oakham, Pepperell, Princeton, Rutland, Shirley, Sterling, Templeton, Townsend, Westminster, and Winchendon), and contiguous towns that are part of the North Central MA healthcare systems. A special emphasis will be placed on communities most disproportionately affected by the social determinants of health.
