Steering Committee Nominations

Steering Committee nominations are currently being accepted. Follow the steps below if you would like to nominate someone for the CHNA9 Steering Committee.

1. Download the CHNA9 Steering Committee Nomination Form (DOC). 
2. Fill out the form completely.
3. Upon completion of the form, email it as an attachment to CHNA 9 Executive Director Chelsey Patriss.
