Active Life Health Care Management is an Adult Day Care Center that provides care and companionship for seniors who need assistance or supervision during the day in the Fitchburg area. We are a community-based program designed to meet the needs of adults with psychiatric, physical, and/or cognitive impairments and chronic health conditions.
Active Life provides for your loved ones with a comfortable, secure, friendly, and faith-based community with whom they can spend their days. Our program promotes the enrichment of the mind, body, and spirit through compassionate care. Care is given without regard to sex, age, handicap, religion, or national origin. Transportation is provided and services are free to qualified MassHealth recipients.
Aging Services of North Central Massachusetts (formerly known as Montachusett Home Care) provides comprehensive information and quality services so that older people, individuals with disabilities, and their families are empowered to make personalized choices to ensure a life of dignity, safety, and respect. Coupled with our commitment to continuous quality improvement and service excellence is our over 45 years of experience in the aging and home care fields. Aging Services of North Central Massachusetts offers comprehensive assessments and coordination of quality long-term care, which is consumer-oriented, cost-effective, and supportive of the autonomy and well-being of the elderly and disabled.

Community Health Connections is offering the Moderna, Pfizer, and Johnson and Johnson vaccines. COVID-19 Vaccination Appointments can now be self-scheduled on the Community Health Connections' website. Same-day appointments may be available - make your appointment today! Children ages 12 to 17 will require a signed consent form and must have an adult present during their appointment.
